What is Defect Eimintion?

This is a structured process in which defects are analyzed and corrective action is implemented to prevent future occurrences.

A Defect Elimination program is a structured process adopted by organisation to become more consistent and reliable in eliminating defects. This is a systematic approach to consistently apply defect elimination across the operations of an organisation, in case of any of any opportunities worthy of attention. Root cause analysis is at the core of such a program and is used to identify the ‘root cause’ of a defect.

The output of the DE process is an action oriented A3 pack with roles, responsibilities and implementation timelines.

The DE Process also prioritises problems into 3 levels depending on severity and hence level of detailed analysis;

  1. Level 1: Minor – 5 WHY
  2. Level 2: Recurring problem – A3
  3. Level 3: Serious problem – Full RCA or FMEA
